How to get your vital documents

Michigan Birth Record
Photo Identification is required to request a Michigan Birth Record. A certified copy is printed on special safety paper and contains a raised, embossed seal. Records are available as early as 1867, however, the initial registration year varies by the county where the birth occurred. Birth records are restricted in Michigan and only a person or parent named on the record or a legal guardian of the person named on the record may request a copy. A legal representative is eligible to request a copy if he/she represents the person named on the record.

Visit the Michigan Department of Community Health website for more information and an application.

Michigan Personal Identification Card
A personal identification card (PID) can be issued to any eligible resident, regardless of age because there is no age requirement. To be eligible, you must provide proof of identification and not possess a valid Michigan driver license. All identification is subject to Secretary of State approval. The cost of a PID is $10. Apply at any Secretary of State branch office.

A person applying for a PID must be photographed. An infant or minor child who is unable to sign their application may have a parent or legal guardian present to sign their application. PID cards issued to applicants under twenty-one years of age have "UNDER 21" printed on the card, above their photo. The PID is valid for four years from the person's last birthday.

Under Michigan law, a PID is as valid as a driver license for identification purposes.

Visit the Secretary of State website for more information and a list of Secretary of State branch offices where you can apply.

Social Security Card
A Social Security number is important because you need it to get a job, collect Social Security benefits and receive some other government services. Many other businesses, such as banks and credit companies, also ask for your number.
