Admission to the Grand Rapids Home for Veterans is accomplished through an application completion process. A completed application form must be forwarded to the Grand Rapids Home for Veterans for eligibility and medical review. Once an individual care level has been determined, the application is returned to the admissions office for processing.

The applicant will be contacted by the admissions office and a date for admission will be set up, or placement on a waiting list based on bed availability will occur.

Obtain application forms by contacting the Admissions Office, which also has a supply of application forms available upon request.
