Must be an Active provider approved for at least 1 course under the Michigan application process.

Submit the following:

CER form
Required filing fees and fee processing card.
Home state approval less than 2 years old.
Detailed content outline for classroom course work approved by the home state.
Detailed table of contents and one copy of the course materials for self-study course work.

Michigan may reduce the home state’s credit hour approval should the course work not meet Michigan’s guidelines for awarding credits for self-study programs. Providers will be allowed to appeal all decisions under Michigan’s guidelines for course approval.
