The agency must obtain a Federal Employer Identification Number (FEIN) where applicable and register with the appropriate government agency. A business entity must meet certain name requirements prior to approval. In its name, an agency may not use the word "insurance" without it being immediately followed by the word "agency". Also, an agency may not use the term "insurance company", which might give the appearance of the entity being an underwriter.
An individual who is licensed in Michigan and holds the qualification for which the agency is applying must be affiliated with the agency prior to licensure. The individual may hold a resident or non-resident license. If the individual is not licensed, refer to Producer/Individual Licensing
To license a Title Agency, follow the same procedures used to license the appropriate business type.
Residents submit the following:
· Application/License
fee - $10.00 check or money order payable to the State of Michigan
· FIS 0223 Fee Card
· Form FIS 0202. Specify type of entity and requested qualification on page 2 of the application.
· Corporation or Limited Liability Company - no organization papers necessary
· Limited Liability Partnership and Partnership - copy of the partnership agreement that was filed with the appropriate government agency.
· Sole Proprietor ship - dba filing.
Non-residents submit the following:
· Application/License
fee - $10.00 check or money order payable to the State of Michigan
· FIS 0223 Fee Card
· Uniform Application for Non-Resident Business Entity Insurance License/Registration - NOTE: Specify type of entity and requested qualification on page 3 of the application.
· Corporation or Limited Liability Company - Articles of Incorporation or Articles of Organization if not licensed in home state.
· Limited Liability Partnership or Partnership - copy of the partnership agreement that was filed with the appropriate government agency if not licensed in home state.
· Sole Proprietorship - dba filing.
