When a licensee, registrant, or applicant decides to cease its Michigan operations, use the MU1 Form, page 4 to notify the Office of Financial and Insurance Regulations (OFIR). Identify the "surrender/cancel" box and complete items 1 (A), and 2. Return the original license/registration document to OFIR along with this form.
