25. Who applies for the birth registration for the newborn?
 
Answer:
Hospital personnel will complete the birth registration form for newborns who are delivered and surrendered in the hospital. For all other surrenders, the child placing agency will make application for the birth registration. Reporting of the birth will be handled as a "foundling" registration. The birth certificate must be completed and filed with the state registrar within five (5) days after assuming custody of the newborn.
