Renewal Information

Renewal packets will be sent three months before the expiration date of your registration/license. 

Family and Group Child Care Homes 

The following renewal application materials will be sent and must be completed to renew the family home registration and the group home license: 

  • Child Care Application (BCAL-3970). 
  • Statement of Registration (BCAL-3909). 
  • Supplemental Application Information (BCAL-3737). 
  • Licensing Record Clearance Request (BCAL-1326). Only required at renewal for household members who have turned 18 years of age or new adult household members who have not previously submitted the DHS-1326. 
  • Medical Clearance Request (BCAL-3704). One for you and any assistant caregivers, signed by your physician (or your assistant caregiver's physician). The Patient Information section must be completed before submitting the form to your (or your assistant caregiver's) physician. 
  • Proof of a current TB test results for any assistant caregivers and any new persons over 14 years of age residing in your home who have not previously submitted documentation.  
  • Proof of valid infant/child/adult CPR and first aid training for you and any assistant caregivers. (See www.michigan.gov/michildcare  for a list of approved organizations for CPR and first aid training.) Proof of blood-borne pathogen training for any assistant caregivers, if not previously provided.
  • Proof of inspection and approval of your heating system (includes wood-burning stoves and any other permanently installed heating devices) and fuel-fired water heater. Note: Electric heat does not require an inspection. Furnaces, other flame or heat-producing equipment used to heat the home when children are in care and fuel-fired water heaters must be inspected by one of the following entities: 
    - A licensed heating contractor for a fuel-fired furnace. 
    - A licensed heating contractor or licensed plumbing contractor for a fuel-fired water heater. 
    -
     A mechanical inspector for the local jurisdiction or licensed mechanical inspector for a wood stove or other solid fuel appliance 
  • The Training Record (BCAL-4590) form and verification of 10 clock hours of training per year by the registrant/licensee and 5 clock hours of training per year by each assistant caregiver, if applicable. Note: Annual training is assessed by the calendar year. Three years' worth of training for family homes and two years' worth of training for group homes must be documented on the BCAL-4590 and verification available.
  • Check or money order payable to the State of Michigan. The renewal fee is $25 for family homes and $50 for group homes. 

Child Care Centers 

The following renewal application materials will be sent and must be completed to renew the center license: 

  • Child Care Application (BCAL-3970). 
  • Supplemental Information Child Care Center (BCAL-3601). 
  • Electronic fingerprint clearance for applicant , each partner, officer, or manager of a child care center using the Licensing Record Clearance (BCAL-1326) if not previously completed for that person. 
  • Child Care Center Designee (BCAL-5003), if applicable and if not previously completed.
  • Staffing Plan (BCAL-5001). 
  • No change in Building Construction Declaration (BCAL-2129). 
    Note:  If you made changes to your building, you cannot use this form. Please contact your consultant for instructions. 
  • Self-Certification of Transportation Rules (BCAL-5044), if applicable.
    Note: This form is not required if transportation is provided in a school bus by a school. [R 400.5602(2)]
     
  • Inspection of fuel-fired furnace by a licensed heating contractor. 
  • Inspection of fuel-fired water heater by a licensed heating contractor or licensed plumbing contractor. 
  • Fire safety inspection of your facility every other renewal (every four years) if your program is not located in a school building. If your program is located in a school building, complete the School-Building Fire Inspection Certification (BCAL-5043). 
  • Environmental Health Inspection Request (BCAL-1787) if you have any of the following:
    - You have private well water and/or septic system.
    Note:  If the center has private water and its water has been tested within the past 12 months, a copy of water testing results can be accepted in lieu of an environmental health inspection unless the center is licensed for food service and/or has a private septic.
    - You provide food service.
    Note:  If a center has a kitchen licensed by the health department, the most recent food service inspection can be accepted in lieu of an environmental health inspection unless the center has private well water and/or septic system.  
     
  • Check or money order payable to the State of Michigan. The renewal fees are as follows:
