Modifying the language and/or instructions on the provided application form could result in rejection of the application, forfeiture of the fee, and possible legal action.
Adobe Reader or Acrobat Version 5 or higher is required to open the
PDF application. The latest version of Adobe Reader is available free from
Adobe
. If the form cannot be saved after it opens, right click on the link and select "Save Target As..." When printing this form, uncheck "Shrink oversized pages to paper size" (Adobe Reader Version 5) or select Paper Scaling: None (Adobe Reader Version 6) in the Print dialogue box so that the application prints at the correct size.

We advise you to refer to, or print out, the Application Appendices for Sample drawings, also located on the main menu, www.michigan.gov/jointpermit. Refer to the application for a listing of sample drawings associated with your project. The location maps, plot plan, and cross-sections, along with the fee
