Project: The Healthy Start/Healthy Families Oakland's Family Connections program will serve 30 at-risk families annually. Potential families, referred through an established network of hospitals, health care providers and community agencies, are first screened across 13 separate risk areas to determine who will be screened out to receive Home Visitation, Family Resource and Support Centers, and/or Other Parent Support Group program prevention services.
