Business Contact Entry
Enter the information to add a contact for the business
- Some fields may auto-populate
- Fields with an asterisk are required
- Step 1: Select Contact Type (**This field may auto-populate.)
- Step 2: Enter Full Name (**This field may auto-populate.)
- Step 3: Enter DOB in mm/dd/yyyy format
- Step 4: Enter address for contact. Use Address 2 line, if applicable
- Step 8: Enter Zip code, include +4 if known
- Step 9: Enter contact email address (**This field may auto-populate.)
- Step 10: Retype email address. Do not copy/paste.
- Step 11: Check box if you would like to request email notifications related to this business be sent to this contact
- Step 12: Enter contact's phone number
*The option to grant account access will be available if you are adding a contact from the business profile.
Click "Save" to save completed contact information (You will be unable to save until all required information has been entered.)
Click "Cancel" to return to previous page without saving.
