Annual Reports of Solid Waste Landfilled in Michigan
Part 115, Solid Waste Management, of the Natural Resources and Environmental Protection Act, 1994, PA 451 as amended, requires all landfills to annually submit a report of the amount of solid waste received by the landfill during that year and, to the extent possible, the origin of that waste by county, state, or country. This report is to be submitted to the Department of Environmental Quality (DEQ) and the county and municipality where the landfill is located. PLEASE NOTE: This form shall be used by all landfill types to determine Perpetual Care Fund deposits; solid waste surcharge payments; and waste receipt data as required by Part 115, Solid Waste Management, of the Natural Resources and Environmental Protection Act, 1994 PA 451, as amended. Please save a copy of this form to an appropriate location on your computer to use for the entire fiscal year (October 1 through September 30).
